Why frozen shoulder is not a stiff muscle.

A frozen shoulder is a problem of the capsule around the joint, not the muscles over it. That is why rubbing the shoulder harder achieves nothing, and why the restriction is still there when somebody else lifts your arm for you. The capsule has tightened, and it changes over time on its own schedule.

It also moves through phases. Pain leads at first. Stiffness takes over as the pain settles. Movement comes back gradually after that. What treatment should be doing is different in each one, and working the wrong phase hard is how people end up more sore and no freer.

So this is not sold as a fix in a fixed number of weeks. It is a course of work matched to the phase you are in, reviewed as that phase changes.

What a frozen shoulder actually is.

Frozen shoulder, also called adhesive capsulitis, is a thickening and tightening of the capsule that surrounds the shoulder joint. Because the restriction is in the joint itself rather than the muscle around it, the movement is limited whether you lift the arm yourself or somebody lifts it for you. That is the clearest thing separating it from a rotator cuff problem, where the arm can usually still be moved for you.

Outward rotation is usually the first movement to go. Reaching behind your back, getting an arm into a coat sleeve, or turning the palm outwards with the elbow tucked in are where people notice it first. Pain at night, particularly lying on that side, is common early on.

It is seen more often in people with diabetes and in people with thyroid conditions, and it can follow a spell of the arm being kept still after an injury or an operation. If none of those has been looked at, your assessment is a sensible prompt to get it checked.

Common signs of frozen shoulder

  • Losing outward rotation first: reaching behind your back, or into a coat sleeve
  • Night pain, often worst lying on that side
  • Restriction that is there whether you move the arm or somebody else does
  • Pain that came on without an obvious injury
  • A shoulder that has been getting stiffer over weeks or months

When a frozen shoulder needs a doctor.

A shoulder that locked up straight after a fall or a dislocation, pain alongside a fever or feeling unwell, or an arm you genuinely cannot move needs medical review rather than soft tissue treatment. Frozen shoulder is also seen more often alongside diabetes and thyroid conditions, so if neither has been checked we will suggest you speak to your GP. We screen at your assessment and refer you on when that is the right call.
